“CORE TRAINING COURSE IN VIBROMASSAGE WITH ELEMENTS OF VIBROTHERAPY”
50 academic hours: 25 hours of theory + 25 hours of practice
The graduate of the core course is able to safely perform apparatus vibromassage on the main body zones, select dosing, consider contraindications, monitor the client’s reaction, and create an individual protocol within the limits of their professional qualification.
This training program is intended as a continuation of the basic course in apparatus vibromassage. Unlike the introductory 32-hour program, the core course deepens work with body zones, functional disorders, myofascial structures, and safe use of elements of local vibrotherapy.
The main task of the course is to develop not only an understanding of the physiological effect of vibration, but also stable practical competence: the student must be able to choose the zone, mode, method, direction of device movement, duration of exposure, and criteria for stopping the procedure.
Professional boundary For students without medical education, the course is limited to wellness and recovery-oriented use without diagnosis and without treatment of diseases. Medical indications, work with diagnoses, rehabilitation after injuries, neuropathies, arthroses, chronic pain, and fibrosis require appropriate qualification and, when necessary, a physician’s prescription.

Course goal, objectives, and pedagogical logic
Program goal
To form professional knowledge, abilities, and practical skills in students for the safe use of apparatus vibromassage in the main anatomical zones and functional tasks: foot, knee, lumbosacral zone, spine and back, neck, shoulder girdle, arms and hands, myofascial trigger zones, fibrotic and scar-related restrictions, chronic pain, and increased tissue sensitivity.
Program objectives
- Deepen the understanding of the physiological effect of mechanical vibration on receptors, muscles, blood vessels, lymph flow, and nervous regulation.
- Teach how to dose exposure by frequency, amplitude, pressure, time, contact area, and the client’s reaction.
- Consolidate stable, labile, longitudinal, segmental, and local methods of apparatus vibromassage.
- Master practical protocols for the foot, knee, lumbar zone, back, neck, arms, and hands.
- Teach how to distinguish an acceptable training situation from a situation requiring refusal of the procedure or referral to a physician.
- Develop the skill of completing a procedure record and analyzing the client’s reaction.
- Prepare the student to create an individual safe protocol for a specific goal.
Pedagogical logic
- First, the basics, safety, and professional boundaries are reviewed.
- Then the physiology of vibration, pain, receptor regulation, and dosing are studied.
- After that, the student masters zones from bottom to top: foot, knee, lumbar zone, back, neck, and upper limbs.
- The final modules focus on complex situations: trigger zones, fibrosis, chronic pain, increased sensitivity, and final protocols.

Abilities
- Conduct an initial contraindication interview and identify red flags.
- Choose the goal of the procedure: relaxation, recovery, preparation, local work, cosmetic care.
- Choose the method: labile, stable, longitudinal, segmental, or combined.
- Select the attachment and mode according to the zone, age, sensitivity, and task.
- Create a protocol for the foot, knee, lumbar zone, back, neck, arms, and hands.
- Monitor the client’s reaction using a sensation scale and objective signs.
- Stop the procedure in case of pain, dizziness, weakness, numbness, or worsening symptoms.
- Complete the procedure record: zone, time, method, attachment, mode, reaction.

Detailed structure of eight modules
Module 1. Review of the basics, safety, and professional boundaries - 6 hours
Goal: Check the student’s readiness for the core course, review basic techniques, and consolidate the safety system.
Theory
- Review of the basic 32-hour course: device, attachments, stable and labile methods.
- Difference between wellness vibromassage and medical vibrotherapy.
- Absolute and relative contraindications.
- Red flags: acute pain, swelling of unknown origin, numbness, weakness, fever, inflammation, thrombosis, oncology, recent injury.
- Professional boundaries for different categories of students.
Practice
- Checking the device, attachments, cord, and workplace.
- Review of labile, stable, and longitudinal techniques.
- Control of pressure force and contact angle.
- Training immediate procedure stop.
- Filling in the training client record.
Module outcome: The student is allowed to work on the main zones only after confirming safe grip, gentle start, ability to stop the procedure, and knowledge of contraindications.
Module 2. Physiology of vibration, receptors, pain, proprioception, and dosing - 6 hours
Goal: Give the student a working understanding of the mechanism of vibration and dosing principles.
Theory
- Mechanical vibration as a physical factor.
- Mechanoreceptors of the skin and deep tissues.
- Proprioception, muscle tone, and nervous system response.
- Acute, chronic, myofascial, and neuropathic pain.
- Dosing parameters: frequency, amplitude, pressure, area, duration.
Practice
- Comparison of gentle and medium intensity on a safe zone.
- Assessment of the client’s reaction on a 0-10 scale.
- Practice of 3-minute gentle local exposure.
- Combination: general pass - local zone - general pass.
- Recording tissue reaction before/after.
Module outcome: The student understands that the power of the device is not equal to effectiveness; the basis of safety is dosing and the person’s reaction.
Module 3. Foot, ankle, lower leg, plantar fascia, and neuropathic risks - 6 hours
Goal: Master safe work with the foot and lower leg, taking into account the plantar fascia, Achilles zone, vascular and neuropathic risks.
Theory
- Anatomical landmarks of the foot and lower leg.
- Plantar fascia, heel, Achilles tendon.
- Gastrocnemius and soleus muscles.
- Plantar pain and work restrictions when inflammation is suspected.
- Diabetic foot, reduced sensitivity, ulcers, and cracks as high-risk zones.
Practice
- Vibromassage lines of the sole.
- Gentle work with the gastrocnemius muscle along the fibers.
- Work around the Achilles zone without strong pressure.
- Protocol “foot and lower leg after load” for 15 minutes.
- Error analysis: pressure on the heel, aggressive work on the fascia, ignoring numbness.
Module outcome: The student can create a gentle protocol for the foot and lower leg and knows when the procedure must not be performed.
Module 4. Knee joint, thigh, muscles around the knee, arthrosis-related and post-load conditions - 6 hours
Goal: Teach how to work with the knee zone through the muscles of the thigh and lower leg without overloading the joint space and popliteal area.
Theory
- The knee as a functional joint-muscle unit.
- Quadriceps, posterior thigh, gastrocnemius muscle.
- Why basic practice should work with the muscles around the knee, not aggressively on the joint.
- Popliteal area as a zone where strong pressure is prohibited.
- Osteoarthritis, post-traumatic and postoperative conditions - only within professional qualification.
Practice
- Lines of the anterior, posterior, and lateral thigh.
- Processing the quadriceps.
- Processing the muscles of the posterior thigh.
- Gentle work around the knee without pressure on the joint space.
- Protocol “knee through thigh muscles” for 20 minutes.
Module outcome: The student can build a knee protocol through the muscular system and does not work aggressively on joint and vascular zones.
Module 5. Lumbar zone, sacrum, pelvis, spine and back: segmental approach - 8 hours
Goal: Master an expanded protocol for the back, lumbosacral zone, pelvis, and paravertebral lines.
Theory
- Lumbosacral zone, gluteal muscles, thoracolumbar fascia.
- Chronic nonspecific low back pain and limitations of non-medical practice.
- Radicular pain and red flags: leg weakness, urination disorders, perineal numbness, fever, trauma, history of oncology.
- Paravertebral lines and segmental principle.
- Work along the spine without pressure on spinous processes.
- Inter-scapular zone, latissimus muscles, thoracolumbar transition.
Practice
- General back protocol for 20-25 minutes.
- Lumbar paravertebral lines.
- Sacral and gluteal zone.
- Gentle work on the thoracolumbar fascia.
- Segmental vibromassage of the back.
- Practicing completion of the procedure and assessment of the client’s reaction.
Module outcome: The student can safely build a back and lumbar protocol, does not press on the spine, and recognizes symptoms requiring medical assessment.
Module 6. Neck, shoulder girdle, arms and hands: safe work with neurovascular zones - 6 hours
Goal: Teach gentle work with the neck-collar zone, shoulder girdle, forearm, and hand.
Theory
- Neck-collar zone: upper trapezius, levator scapulae, suboccipital zone.
- Prohibited and dangerous zones: anterior neck surface, carotid arteries, pronounced dizziness.
- Shoulder girdle and scapular zone.
- Forearm: wrist flexors and extensors.
- Carpal tunnel, cubital tunnel, finger numbness, and neuropathic risks.
- Hand overload syndrome and restrictions on aggressive vibration.
Practice
- Protocol “neck and shoulder girdle” for 15 minutes.
- Gentle work with the upper trapezius.
- Scapular zone and deltoid area.
- Forearm lines: flexors and extensors.
- Gentle work with the hand without pressure on the carpal tunnel.
- Stopping the procedure in case of numbness, increased pain, or dizziness.
Module outcome: The student can work gently, briefly, and precisely; intensive vibration is not used in vascular and nerve zones.
Module 7. Myofascial trigger zones, fibrosis, scars, chronic pain, and fibromyalgia - 6 hours
Goal: Teach cautious local work with complex soft-tissue conditions without medical promises and without provoking exacerbation.
Theory
- Myofascial pain and trigger zones: the concept of a muscle band and local tenderness.
- Difference between a trigger zone and joint, neurological, or inflammatory pathology.
- Fibrosis, scars, adhesions, and soft-tissue restrictions.
- Work with a scar zone only after complete healing.
- Fibromyalgia and chronic pain as states of increased sensitivity.
- Principle: lower intensity - more control.
Practice
- Gentle tissue reaction test.
- Local stable technique for 5-15 seconds with pain control.
- Combination: general pass - local zone - general pass.
- Work around the scar, not aggressively on the scar.
- Protocol for high sensitivity for 10-15 minutes.
- Error analysis: deep vibration, prolonged holding, work on inflammation.
Module outcome: The student can use local work cautiously, controls pain, and does not use aggressive techniques in case of increased sensitivity.
Module 8. Creation of professional protocols, clinical tasks, and final exam - 6 hours
Goal: Consolidate the student’s ability to independently create and perform safe protocols according to a given goal.
Theory
- Structure of a professional protocol: goal, zone, attachment, method, mode, time, control, completion.
- Algorithm for choosing the zone and mode.
- Training clinical tasks: foot, knee, lumbar zone, neck, hand, trigger zone, high sensitivity.
- Procedure documentation.
- Legal and professional caution.
- Preparation for final assessment.
Practice
- The student chooses a training task and creates a protocol.
- Conducts a short contraindication interview.
- Performs the procedure for 15-25 minutes.
- Completes the procedure record.
- Receives instructor feedback.
- Passes the practical exam.
Module outcome: The student confirms readiness for independent work within the limits of their qualification.
Learning control and final assessment

Criteria for the practical exam
- Safety and contraindications — 0-5 (Minimum requirement: correctly identifies restrictions and does not start a dangerous procedure)
- Device technique — 0-5 (Minimum requirement: gentle start, even contact, without excessive pressure)
- Dosing — 0-5 (Minimum requirement: adequately chooses time, zone, attachment, and intensity)
- Communication and reaction control — 0-3 (Minimum requirement: asks about sensations and adjusts the procedure)
- Documentation — 0-2 (Minimum requirement: completes the procedure record)
Grading
18-20 points - confident performance. 16-17 points - pass with minor comments. 12-15 points - repeated practice is required. Less than 12 points - exam not passed.
Requirements for equipment, safety, and documentation
Equipment
- apparatus vibromassager with adjustable modes;
- set of attachments/vibratodes for large, medium, and small zones;
- massage table, disposable sheets, towels;
- attachment cleaning agents and antiseptic;
- timer, procedure record, contraindication table;
- training diagrams of massage lines and risk zones.
Mandatory safety rules
- start with minimum intensity;
- do not work on damaged skin, acute inflammation, recent injuries, thrombosis, or swelling of unknown origin;
- do not use aggressive vibration on the neck, in the popliteal area, in the area of nerve tunnels of the hand, or on joint spaces;
- stop the procedure in case of dizziness, nausea, weakness, sharp pain, numbness, or worsening symptoms;
- record the procedure parameters and the client’s reaction.

- subjective reaction of the client;

- reasons for stopping or changing the protocol, if any.
The main course continues the 32-hour basic programme: deeper work with body zones, functional disorders, myofascial structures, and the safe use of elements of local vibrotherapy.
